Job summary

21st Century Home Health Services (21HHS) is seeking an Occupational Therapist to provide skilled in-home services throughout Monterey County. You’ll assess patients in their homes, address safety, mobility, and functional needs, and collaborate with an interdisciplinary team to support recovery and independence.

The role emphasizes compassionate care, patient advocacy, and education, with a structured bonus plan and comprehensive benefits.

Qualifications

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification is required.
• Valid CA Driver's License required.
• Knowledge of local community resources is required.
• Experience in home health care is highly preferred.

Responsibilities

• Provide skilled OT services in patients' homes in partnership with the Case Manager.
• Evaluate and address home safety, medication management, wheelchair seating and positioning, pain management, and falls prevention.
• Serve as educator, patient advocate, and member of the interdisciplinary care team.
