Qualifications
- Experience in home health care is highly preferred.
- Understanding of individual development and human behavior related to illness/injury.
- Experience working with interdisciplinary teams is highly desired.
Responsibilities
- Provide skilled services to evaluate home safety, medication management, and chronic disease management.
- Serve as an educator, patient advocate, and part of the interdisciplinary care team.
- Create your own schedule and coverage for patient care.
