Job summary

Matrix Medical Network is seeking experienced Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes across Indiana. Each visit lasts up to one hour and combines health review, history, medication adherence, and risk assessment to close care gaps.

You will collaborate with physicians, case managers, and care teams, travel extensively, and receive flexible scheduling with opportunities to transition to full-time roles.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in relevant NP specialty (Adult/Gerontology/Family).
• Current BLS/ACLS/CPR certification.
• At least 1 year of NP experience.
• Willingness to travel up to 80-90% including overnight stays.
• Valid driver's license and auto insurance.
• Strong computer skills and familiarity with health records software.

Responsibilities

• Perform adult/geriatric health assessments in the member's home or facility.
• Review medical history, medications, and risk factors; document per Matrix guidelines.
• Coordinate with PCPs, case managers, social workers, and caregivers.
• Order/administrate point-of-care and lab tests when needed.
• Provide patient education and follow-up in collaboration with care team.

Excellent verbal and written comm.

Ability to travel 80-90%

Education

Master’s Degree or commensurate NP experience
