Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in member homes across Indiana. Each visit lasts up to one hour and focuses on health status, medical history, medication adherence, and risk factors to close care gaps.

Providers administer testing, collaborate with PCPs and care teams, and deliver education and referrals. Flexible scheduling, travel up to 80–90%, and opportunities for licensure support are offered.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice including prescription authority.
• AANP/AACN/ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience.
• Able to travel up to 80-90% of the time including overnight travel.
• Valid state driver’s license.
• Automobile insurance coverage as required for the state of residence.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult / geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other medical staff as appropriate.
• Coordinate with Primary Care Physician on patient education and follow up as necessary.
• Provide services at the member’s home or Skilled Nursing Facility.
