Job summary

Community Health Network is seeking an Advanced Practice Provider to join the Community ProCare at Home program in Indianapolis, IN. Qualifications

• APN must be licensed for Indiana.
• Experience with chronically ill patients preferred.
• House call/home care experience preferred.

Responsibilities
