Job summary

You will perform clinical assessments, develop personalized care plans, and coordinate with interdisciplinary teams to ensure smooth transitions and preventative care.

Qualifications

• Active state NP license required.
• Current NP licensure.
• Master’s degree in Nursing (MSN).
• Five+ years nursing experience; NP experience preferred.
• Strong interpersonal and written communication skills.
• Ability to manage daily workflow and time.
• Proficiency with medical instruments/equipment.
• HIPAA/OSHA knowledge.
• Experience with EMR systems and Microsoft Office.

Responsibilities

• Perform clinical assessments and develop care plans.
• Treat chronic and acute conditions in home settings.
• Order and interpret labs and diagnostics.
• Collaborate with caregivers and interdisciplinary teams.
• Provide referrals as needed.
• Maintain up-to-date patient records.
