Job summary

Matrix Medical Network delivers in-home health assessments across the nation, helping seniors and at-risk individuals maintain quality of care. Providers review health status, medication adherence, and social factors during visits at the member’s home, often in home health settings or SNFs.

As a clinician with NP licensure, you’ll perform comprehensive CHA assessments, coordinate with PCPs, and support patient education and follow-up to close care gaps.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP license for the state of practice with prescriptive authority or ability to obtain.
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family, Emergency, or Acute Care specialties.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and other care team members.
