Job summary

Matrix Medical Network deploys NP-led in-home health assessments to older adults and at‑risk individuals, aiming to improve outcomes and close care gaps. Providers review health history, medications, and living environment during one-hour visits, then coordinate with PCPs and care teams to tailor follow-up care.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in a relevant NP specialty (Adult/Family/Gerontology).
• BLS/ACLS certification and clean driving record.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers for care planning.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
