Job summary

Matrix Medical Network delivers in-home health assessments to older adults and at-risk individuals, aiming to improve health outcomes and close care gaps. Providers perform comprehensive reviews in patients' homes or SNFs, including medical history, testing, and education, while collaborating with physicians and care teams.

We offer flexible scheduling across Sundays to Saturdays, with earnings based on the number of assessments completed and a per-assessment rate.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in applicable NP fields.
• BLS/ACLS/CPR certification is required.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance coverage.
• Strong computer skills and familiarity with EMR software.

Responsibilities

• Conduct adult/geriatric health assessments, medical history, and follow-up.
• Coordinate with PCPs, case managers, and family members for care.
• Order and administer point-of-care or lab tests as needed.
• Provide home-based health services at members’ residences or SNFs.
• Document assessments accurately per Matrix guidelines.
