Matrix Medical Network seeks NP providers to perform comprehensive in-home health assessments, reviewing medical history, meds, and social factors to identify care gaps. You will coordinate tests and work with PCPs and care teams on follow-up, delivering patient education in patients’ homes or SNFs.
Providers enjoy flexible scheduling, with visits scheduled around availability, and compensation tied to assessments completed.
Competenze
- Must hold NP licensure in practicing state with prescriptive authority
- Board certification in Adult/Family/GERIATRIC or related specialty
- Current BLS/ACLS/CPR certification
- Minimum 1 year NP clinical experience; new grads welcome
- Valid state driver’s license and auto insurance coverage for residence state
Mansioni
- Perform comprehensive adult/geriatric health assessments in home settings
- Review medical history, medications, social factors; document per Matrix standards
- Order and coordinate point-of-care and lab testing as needed
- Collaborate with PCPs, case managers, and caregivers for follow-up
- Provide patient education and follow-up in residence or SNF
