Job summary

Matrix Medical Network in the United States is staffing Nurse Practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications, and living environment. Providers conduct geriatric assessments, order tests, coordinate with PCPs, and document findings according to Matrix guidelines, delivering care at members' homes or SNFs.

Qualifications

• Must hold NP licensure in state of practice with prescriptive authority.
• Board certification in Adult/Gerontology Primary Care, Family, or related field.
• Current CPR/BLS/ACLS certification is required or obtainable.
• Strong computer skills and experience with medical record software.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnosis and treatment planning.
• Order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, family members, and caregivers.
• Provide services at member residences (homes or Skilled Nursing Facilities) and document per Matrix guidelines.
