Job summary

Matrix Medical Network seeks skilled nurse practitioners to perform comprehensive geriatric assessments during home visits. You will review medical history, medications, and risk factors, with documentation per Matrix guidelines.

The role offers flexible scheduling and in-home services at members' residences or SNFs, with opportunities for collaboration with PCPs and care teams across the network.

Qualifications

• > Master’s Degree or commensurate experience in nursing; NP licensure.
• "> Current NP licensure in state of practice; prescriptive authority as needed.
• "> Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care NP specialties.
• "> BLS/ACLS/CPR certification required.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and primary care teams.
• Provide in-home services at members' residences or Skilled Nursing Facilities.
• Coordinate care and follow-up with PCPs as necessary.
