Job summary

Matrix Medical Network is seeking experienced Nurse Practitioners to perform comprehensive in-home health assessments for adults and seniors. You will review medical history, medications, and risk factors, and coordinate with physicians and care teams to close care gaps.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in the state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult/Adult Gerontology/Family/ Emergency/ Acute Care NP specialties.
• Current BLS, ACLS or CPR certification.
• Minimum 1 year NP experience.

Responsibilities

• Conduct adult/geriatric assessments including medical history and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Educate patients and coordinate follow-up with PCPs.
