Job summary

Matrix Medical Network conducts comprehensive in-home health assessments for members across the nation. Providers review health history, medications, and risk factors during visits that may last up to one hour, with services at the member’s residence.

We offer flexible scheduling, competitive pay per assessment, and opportunities for additional licensure and state creden tials. Travel up to 80-90% is common, with support for onboarding and ongoing education.

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Current NP licensure in the state of practice with prescriptive authority if needed.
• AANP/AACN/ANCC board certification in one of listed adult/gerontology/family specialties.
• Current BLS/ACLS or CPR certification.
• Minimum 1 year NP experience.
• Ability to travel up to 80-90% including overnight travel.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, and other healthcare staff.
• Provide services at the member’s residence, including homes or SNFs.
