Job summary

Matrix Medical Network is seeking a seasoned Nurse Practitioner to perform Comprehensive Health Assessments in members' homes or facilities, working with a multidisciplinary team to review health history, medications, and risks. You will provide education, referrals, and follow-up while traveling across assigned states.

The role requires 1 year of NP experience, current NP licensure with prescriptive authority, and the ability to travel extensively.

Qualifications

• Master’s Degree in Nursing or commensurate experience with NP licensure
• Current NP licensure in state of practice with prescriptive authority as needed
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family, Emergency or Acute Care
• Current BLS/ACLS or CPR certification
• 1 year of NP experience
• Ability to travel 80-90% of the time including overnight travel
• Valid driver’s license for state of residence and automobile insurance coverage
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals and documentation
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers and family
• Coordinate with PCP on education and follow-up
• Provide services at member residences including homes or skilled nursing facilities
