Job summary

Matrix Medical Network conducts in-home comprehensive health assessments to improve care quality and close care gaps. Providers perform geriatric assessments, coordinate with physicians, and deliver services at patients' homes or skilled nursing facilities, with attention to health history, adherence, and social determinants.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice
• Board certification in Adult/ Gerontology Adult/Family/Acute Care/ Emergency
• Current BLS/ACLS or CPR certification
• 1 year NP experience preferred; new grads welcome

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnosis
• Administer and order point-of-care and lab testing
• Collaborate with physicians, case managers, social workers and family
• Provide education and follow-up with PCP
• Provide services at member residences (home or SNF)

Skills

NP licensure

Board certification (Adult/ Geront.)

Excellent communication

Driving license

Computer skills with medical records

Education
