Job summary

Matrix Medical Network is hiring Nurse Practitioners to perform comprehensive in-home health assessments for adults, including geriatrics. You will review health history, medications, and social factors, and coordinate with PCPs for follow-up.

Positions are full-time with flexible scheduling. You will travel to members’ homes or Skilled Nursing Facilities, conducting assessments and documenting findings to improve care outcomes.

Qualifications

• Master’s degree and NP licensure in state of practice.
• Current NP licensure with prescriptive authority or ability to obtain.
• Board certification in relevant NP specialty.
• BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and other providers.
• Coordinate with PCP on patient education and follow-up visits.
• Provide services at members’ residences, including homes or SNFs.

Skills

Excellent verbal and written comms

Strong computer skills

1 year NP experience preferred

Education

Master’s Degree in NP or equivalent
