Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments. You will review medical history, medications, functional status, and care gaps, documenting findings to improve care quality.

Role involves collaboration with PCPs, case managers, and caregivers, plus patient education and follow-up planning. In-home visits can be at patients’ homes or SNFs; flexible scheduling supports work-life balance.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Certification by AANP/AACN/ANCC in appropriate NP specialty.
• BLS, ACLS or CPR certification current.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance coverage.
• Familiarity with health record software and patient education.

Responsibilities

• Conduct adult/geriatric in-home health assessments.
• Review medical history, medications, and care gaps.
• Coordinate with physicians, case managers, and caregivers.
• Provide patient education and follow-up planning.
• Document findings per Matrix guidelines and protocols.
