Job summary

Matrix Medical Network seeks a licensed Nurse Practitioner to perform in-home comprehensive health assessments. You will review medical history, assess health status, provide patient education, and coordinate with physicians and care teams during home visits across Massachusetts.

The role requires NP licensure in MA, board certification, and ability to travel extensively. Flexible scheduling and strong clinical support are provided.

Qualifications

• Master’s degree in nursing and NP licensure.
• Current NP licensure with prescriptive authority.
• Board certification in Adult, Gerontology, Family, or Emergency specialties.
• BLS/ACLS certification.
• Willingness to travel up to 80-90% including overnights.

Responsibilities

• Conduct adult and geriatric health assessments and document findings.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, and family/caregivers.
• Provide in-home assessments at members’ residences and coordinate follow-up care.
