Job summary

Matrix Medical Network offers in-home comprehensive health assessments conducted by highly qualified Nurse Practitioners. The role covers medical history review, diagnosis, education, and care coordination, with a focus on closing care gaps during each visit.

The position requires NP licensure in the practice state, board certification in related specialties, and strong communication skills. Travel up to 80–90% is expected, with support for licensure and ongoing education.

Qualifications

• Master’s degree or commensurate experience with NP licensure required.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties preferred.
• BLS/ACLS certification required.
• 1 year NP experience preferred, with ability to travel 80–90% in a given state or region.

Responsibilities

• Conduct adult/geriatric assessments including medical history and current health status.
• Administer and order point-of-care and lab-based testing as needed.
• Coordinate with physicians, case managers, social workers, and family members as appropriate.
• Collaborate with PCP on patient education and follow up.
• Provide services at the member’s residence, including homes and Skilled Nursing Facilities.
