Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments. You will review medical history, medications, and social factors to close care gaps and inform treatment plans.

The role involves collaboration with physicians, case managers, and other clinicians, with visits at members’ homes or SNFs. Strong NP licensure and state credentials are required, plus willingness to travel.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in Adult, Family, or Gerontology specialties.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license with automobile insurance coverage.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history and follow-up.
• Administer point-of-care and lab-based tests as needed.
• Collaborate with PCPs, case managers, and care teams for education and follow-up.
• Provide services at the member’s home or Skilled Nursing Facilities.
