Job summary

Matrix Medical Network is seeking NP professionals to conduct comprehensive in-home health assessments for older adults and at-risk individuals. Each visit lasts up to one hour and focuses on medical history, adherence, and social factors to identify chronic conditions and care gaps.

Providers will perform or order tests, collaborate with a care team, and deliver education and follow-up in patients’ homes or in Skilled Nursing Facilities.

Qualifications

• Master’s degree in nursing practice and NP licensure required.
• Active NP licensure in the practicing state with prescriptive authority.
• Board certification in Adult, AGPCN, Family, Emergency, or Acute Care.
• BLS/ACLS/CPR certification current or eligible.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family members as appropriate.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence (home or SNF).
