Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive adult and geriatric health assessments at patients’ homes or Skilled Nursing Facilities across the region.

Providers review medical history, medications and social determinants to close care gaps and improve outcomes. The role offers flexible scheduling, mileage reimbursement and ongoing education, with potential transition to full-time roles.

Qualifications

• Master’s degree in NP or equivalent with licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in related NP specialty.
• BLS, ACLS, or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Auto insurance coverage for residence state.
• Strong computer skills and familiarity with EMR software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric health assessments including history, diagnosis and treatment, education, referrals and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Coordinate patient education and follow-up with PCPs.
• Provide services at the member’s residence (homes or SNFs).
