Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. Each visit lasts up to an hour and includes medical history, medication review, and risk assessment to identify care gaps.

You will collaborate with physicians, case managers, social workers and family members, order tests as needed, and provide education and follow-up to improve health outcomes while working flexible schedules across the

Qualifications

• Must hold NP licensure and prescriptive authority in state of practice.
• Board certification in Adult/Family/Gerontology nursing preferred.
• Current BLS/ACLS or CPR certification required.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Perform Comprehensive Health Assessments in members' homes or SNFs.
• Review medical history, diagnoses, medications, and risk factors.
• Order and interpret tests; coordinate care with PCPs and team.
• Document findings and follow Matrix guidelines and protocols.
