Job summary

Matrix Medical Network seeks licensed NP providers to perform Comprehensive Health Assessments (CHA) in members' homes or SNFs. Each visit lasts up to one hour and informs care gaps, treatment decisions, and patient education.

Providers review medical history, medications, social factors, and health status while coordinating with PCPs and care teams. Hiring is for full-time roles with ongoing onboarding, preceptorship, and potential licensure support across states.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescription authority.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred, new grads encouraged to apply!
• Valid state driver's license for the state of residence.
• Automobile insurance coverage requiring for the state of residence (or ability to obtain)
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up, and documentation.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, family members, and other healthcare personnel.
• Coordinate with Primary Care Physician on patient education and follow-up.
• Provide in-home or Skilled Nursing Facility services at members' residences.
