Job summary

Matrix Medical Network offers in-home Comprehensive Health Assessments (CHA) for members across the nation. The role involves reviewing medical history, medications, and social factors to identify care gaps and improve outcomes.

Providers conduct assessments at members’ residences or facilities, with flexible hours, and potential pathways to full-time roles. Competitive pay and benefits support continued learning and licensure.

Qualifications

• Master’s degree or commensurate NP experience and licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in applicable NP specialty (Adult, Gerontology, Family, etc.).
• Current BLS, ACLS or CPR certification.
• Minimum 1 year NP experience.
• Ability to travel up to 80–90% including overnight trips across states.

Responsibilities

• Perform adult/geriatric health assessments including medical history, diagnoses, and treatment plans.
• Order and administer point-of-care and lab-based tests as needed.
• Coordinate with physicians, case managers, social workers, and caregivers.
• Collaborate with PCP on education and follow-up.
