Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform in-home Comprehensive Health Assessments (CHA) for members, reviewing medical history, medications, and overall health in the member’s home or SNF. Visits can last up to one hour, providing detailed insights that help close care gaps.

The role requires NP licensure in the state of practice, prescriptive authority or ability to obtain, and board certification in a relevant specialty.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult/Adult Gerontology/Family/ Emergency NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP for education and follow-up.
