Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform in-home comprehensive health assessments. You will review health history, medications, and social factors, delivering care at members’ homes or SNFs. The CHA aims to close care gaps and improve outcomes.

Responsibilities include adult/geriatric assessments, testing as needed, collaboration with care teams, and education with follow-up. Flexible scheduling with weekend options is offered, plus training and licensure support.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice including prescriptive authority.
• Board certification in Adult, Gerontology, Family or related NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and automobile insurance coverage.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals and documentation.
• Administer and order point of care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, families and caregivers.
• Educate patients and coordinate follow-up with PCP as necessary.
• Provide services at the member’s residence, homes or Skilled Nursing Facilities.
