Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments, lasting up to one hour, evaluating medical history, medications, and social factors to identify gaps in care. The CHA visits occur in members’ homes or Skilled Nursing Facilities and aim to improve outcomes through targeted care coordination with PCPs and care teams.

Qualifications

• Master’s degree or commensurate NP training; must be licensed as NP.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance for residence.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments to include medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other staff.
• Coordinate with PCP on patient education and follow up.
• Provide services at the member’s home or SNFs.
