Job summary

Matrix Medical Network conducts comprehensive in-home health assessments (CHA) for members, with visits lasting up to an hour. Providers review medical history, current health, medication adherence, and social factors to identify care gaps and coordinate with PCPs.

Roles offer flexible scheduling, including weekends, and compensation based on the number of assessments completed. Opportunities for licensure support and ongoing education are available, with full-time sign-on bonuses for

Qualifications

Master’s Degree in Nursing and NP licensure.
Current NP licensure in the state of practice.
Board certification in Adult, AGPC, AC, Family, Emergency, or Acute Care specialties.
CPR/BLS/ACLS certification.
1 year NP experience preferred; new grads welcome.
Valid state driver's license and auto insurance coverage.
Strong computer skills and familiarity with health/medical record software.
Excellent verbal and written communication skills.

Responsibilities

Assess adult/geriatric patients including medical history, diagnoses, treatments, health education, referrals, and documentation per Matrix protocols.
Administer and order point-of-care and lab-based testing as needed.
Collaborate with physicians, case managers, social workers, family members, and other care team members.
Coordinate with PCP on patient education and follow-up.
Provide in-home or SNF-based assessments at the member’s residence.
