Job summary

Matrix Medical Network is expanding its national in-home health assessment program, employing Nurse Practitioners to conduct comprehensive CHA visits in members’ homes or SNFs. Providers review health history, adherence, and risks to identify gaps in care and coordinate with physicians and care teams.

Visits can last up to one hour and are scheduled based on your availability, including weekends. We offer competitive pay per assessment and support ongoing licensure and training.

Qualifications

• Master’s degree and NP licensure in state of practice.
• Current NP licensure with prescriptive authority.
• Board certification in Adult/Primary Care specialties.
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license for state of residence.
• Automobile insurance coverage for state of residence.
• Strong computer skills with medical record software.
• Excellent verbal and written communication.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnosis, treatment, health education and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or SNF).
