Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments (CHA) across our national network. You’ll review medical history, medication adherence, social factors, and risks to uncover gaps in care during a single visit.

Responsibilities include conducting geriatric assessments, coordinating with PCPs, and providing patient education in homes or SNFs. We offer flexible scheduling, competitive pay, mileage reimbursement, and ongoing professional support.

Qualifications

• Must hold current NP licensure in the state of practice and be able to obtain prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in relevant NP specialties is required.
• BLS/ACLS certification required.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance may be required.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers to coordinate care.
• Coordinate patient education and follow-up with PCP as necessary.
• Provide services at members' homes or Skilled Nursing Facilities.
