Job summary

Matrix Medical Network is recruiting nurse practitioners to perform in-home Comprehensive Health Assessments (CHA). Each visit lasts up to one hour and provides in-depth review of health history, medications, and social factors to identify care gaps and guide treatment.

Providers administer point-of-care testing as needed and collaborate with physicians, case managers, and caregivers to coordinate education and follow-up.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP/AACN/ANCC board certification in an adult-gerontology or family specialty.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses, treatments, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers; coordinate with PCP for education and follow-up.
• Provide services at members’ residences, including homes or skilled nursing facilities.
• Maintain accurate, timely documentation and follow-through on care gaps.
