Job summary

Matrix Medical Network is seeking skilled NP providers to perform comprehensive in-home health assessments. Each visit lasts up to an hour and reviews medical history, medications, and social factors to close care gaps.

You will work with physicians and care teams, conducting assessments at members’ homes or SNFs, with scheduling that accommodates your availability. Matrix offers FT opportunities and growth paths.

Qualifications

• Master’s degree or commensurate NP experience with licensure.
• Current NP licensure in practice state with prescriptive authority if needed.
• Board certification in Adult, Gerontology, Family, or Acute Care specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance.
• Strong computer skills and EMR familiarity.
• Excellent verbal and written communication.

Responsibilities

• Conduct adult/geriatric health assessments with medical history, diagnoses, and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and caregivers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at members’ homes or Skilled Nursing Facilities.
