Job summary

Matrix Medical Network seeks experienced nurse practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors. You’ll work with physicians, care teams, and families to close care gaps and improve outcomes in patients’ homes or Skilled Nursing Facilities.

Responsibilities include conducting adult-geriatric assessments, ordering testing, and coordinating follow-up.

Qualifications

• Master’s degree or equivalent with NP licensure.
• Current NP licensure in the state of practice including prescriptive authority.
• Board certification in Adult, Adult Gerontology, Family NP or related specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance coverage.
• Strong computer skills and familiarity with medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other staff as appropriate.
• Coordinate with PCP for patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
