Job summary

Matrix Medical Network seeks Nurse Practitioners to perform in-home comprehensive health assessments, reviewing medical history, medications, and social factors to identify care gaps. Visits occur at members' residences, including homes and Skilled Nursing Facilities, with a focus on improving health outcomes.

Providers work within Matrix guidelines, with support from clinicians and staff, and receive onboarding, mentorship, and access to continuing education.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice, with prescriptive authority if needed.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
• BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses, treatment, and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on education and follow-up.
