Job summary

Matrix Medical Network offers in-home comprehensive health assessments led by nurse practitioners, visiting members in their homes or skilled nursing facilities. The role focuses on health history, med reviews, risk assessment, and coordinated care delivery to close care gaps during each visit.

Providers work with physicians, case managers, social workers, and family members, with emphasis on education, adherence, and documentation.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management, and thorough documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence, which may be in homes or Skilled Nursing Facilities.
