Job summary

Matrix Medical Network seeks skilled Nurse Practitioners to conduct comprehensive in-home health assessments. You’ll review health history, medications, and risk factors, then coordinate with physicians, case managers, and caregivers to close care gaps during visits at the member's home or in SNFs.

Strong NP licensure, prescriptive authority, and board certification are required along with BLS/ACLS. Flexible scheduling and potential FT opportunities support professional growth across regions.

Qualifications

• Master’s degree in nursing or equivalent experience.
• NP licensure in the practicing state and prescriptive authority as needed.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS/ACLS or CPR certification.

Responsibilities

• Perform adult/geriatric health assessments during home visits.
• Review medical history, medications, and risk factors with documentation.
• Coordinate with PCPs, case managers, and caregivers for follow-up.
• Order point-of-care tests and lab work as needed and document results.
• Provide patient education and ensure care gaps are closed when possible.
