Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform Comprehensive Health Assessments (CHA) in patients' homes or Skilled Nursing Facilities. You will review medical history, medications, and risk factors, and coordinate care with physicians and case managers.

The role requires NP licensure in the state, applicable board certification, and travel up to 80-90% including overnight travel. We offer a sign-on bonus and flexible scheduling.

Qualifications

• Must hold NP licensure in state of practice.
• Board certification in Adult/Family Gerontology or related field.
• BLS/ACLS certification required.
• Must be able to travel up to 80-90% including overnight stays.
• Valid state driver’s license and auto insurance coverage.

Responsibilities

• Conduct adult/geriatric health assessments and document findings.
• Administer and order point-of-care tests and lab work as needed.
• Collaborate with PCPs, case managers and other team members.
• Provide services in patients' homes or SNFs, with clear follow-up.
• Maintain thorough documentation per Matrix guidelines.
