Job summary

Matrix Medical Network is recruiting Nurse Practitioners to perform in‑home Comprehensive Health Assessments (CHA) across the United States. Each visit lasts up to an hour and focuses on health history, medications, adherence, and social factors to uncover care gaps and improve outcomes.

Providers collaborate with PCPs, case managers, and families, delivering education, coordinating follow‑ups, and arranging referrals.

Qualifications

• Master’s Degree in Nursing and NP licensure in state of practice.
• Current NP licensure with prescriptive authority or ability to obtain.
• Board certification in Adult/Family/Gerontology/Acute Care specialties and CPR/BLS certification.

Responsibilities

• Perform comprehensive in-home assessments focusing on medical history, medications, and social factors.
• Order and administer point-of-care and basic lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers to coordinate care.
• Educate patients and families and document findings per Matrix guidelines.
• Provide services at patients’ homes or SNFs as required.
