Job summary

Matrix Medical Network in the United States delivers comprehensive in-home health assessments through a national clinician network. As a Nurse Practitioner, you review medical history, medications, and functional status in patients' homes or SNFs to close care gaps and improve outcomes.

You'll collaborate with physicians, case managers, social workers, and caregivers, performing necessary testing and education.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice with prescription authority or ability to obtain.
• Board certification in Adult/Family/Gerontology NP specialties.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnostics.
• Administer point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers and caregivers.
• Provide services at the member’s residence, homes or SNFs.
• Educate patients and follow up as necessary.
