Job summary

Matrix Medical Network is seeking a skilled Nurse Practitioner to perform Comprehensive Health Assessments in home environments across the United States. Providers review health histories, medications, and risks, and collaborate with PCPs to close care gaps during a single visit.

Qualifications

Master’s degree or commensurate experience with NP licensure
Current NP licensure with prescriptive authority
AANP/AACN/ANCC board certification in Adult, Gerontology, Family or Emergency specialties
BLS, ACLS or CPR certification
1 year NP experience preferred; new grads welcome
Valid state driver’s license with adequate auto insurance
Strong computer skills and familiarity with EHR software
Excellent verbal and written communication skills

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals and case management
Administer and order point-of-care and lab-based testing as needed
Collaborate with physicians, case managers, social workers, family members and other care team
Coordinate with PCP on education and follow-up
Provide services at the member’s home or SNF with clear documentation
