Job summary

Matrix Medical Network is seeking qualified NP clinicians to perform Comprehensive Health Assessments in members’ homes or SNFs. Each visit lasts up to an hour and focuses on health history, medication adherence, and social determinants to close care gaps.

We offer a $1,500 sign-on bonus, flexible schedules, and competitive pay based on the number of assessments completed, with potential monthly earnings from about $1,800 to $11,500. Benefits and licensure support available.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice and prescriptive authority if needed.
• Board certification in related Adult/Family/AGNP specialties.
• BLS/ACLS or CPR certification; valid driver’s license.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers; provide patient education and follow-up.
• Provide services at the member’s residence, which may be home or Skilled Nursing Facilities.
• Support ongoing care coordination and care gap closure via CHA visits.
