Job summary

Matrix Medical Network is hiring NP professionals to perform Comprehensive Health Assessments in members’ homes, supporting quality care and care-gap closure. Each visit reviews medical history, medications, and social context, with documentation per Matrix guidelines.

Positions offer generous sign‑on bonuses, flexible scheduling, and opportunities to transition from PRN to full‑time roles across states. Travel can be substantial, including long durations and weekends.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult/Family gerontology specialties.
• Current BLS/ACLS certification.
• 1 year of NP experience.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers as appropriate.
• Coordinate with PCP for patient education and follow-up.
