Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform Comprehensive Health Assessments in members’ homes or SNFs across the country. You will evaluate medical history, medication adherence, and social determinants to identify care gaps and coordinate follow-up with PCPs.

Responsibilities includeordering tests, charting per Matrix guidelines, and collaborating with a multidisciplinary team to improve outcomes.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain it.
• Boards: AANP, AACN, or ANCC in Adult/Adult Gerontology Primary Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care providers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence, which may be in homes or Skilled Nursing Facilities.
