Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, review medical history, and coordinate care with PCPs and care teams. Visits occur at members’ homes or SNFs, lasting up to an hour, with emphasis on accurate documentation and follow-up.

The role requires NP licensure, board certification, and the ability to travel up to 15% for coverage across locations. A strong team and ongoing education are offered.

Qualifications

• Master’s Degree or commensurate NP experience and licensure.
• Current NP licensure in practicing state with prescriptive authority.
• Board certification in Adult/Family/Gerontology specialties is required or obtainable.

Responsibilities

• Conduct adult/geriatric health assessments and document findings per Matrix guidelines.
• Administer point-of-care and lab tests as needed and coordinate follow-up.
• Collaborate with physicians, case managers and social workers to ensure coordinated care.
• Provide services at member residences, including homes and SNFs.
