Job summary

Matrix Medical Network is seeking an experienced Nurse Practitioner to perform comprehensive in-home health assessments. You will evaluate medical history, medications, and risk factors, and collaborate with physicians, case managers, and families to close care gaps.

Travel up to 80-90% is required, with visits at member homes or Skilled Nursing Facilities. A Master's in Nursing, NP licensure, and board certification are essential, along with valid driver’s license and CPR/BLS credentials.

Qualifications

• Must hold a Master’s degree in nursing and NP licensure in the practicing state.
• Board certification in an NP specialty (adult/gerontology/family) is required.
• Active BLS/ACLS/CPR certification and good driving record required.
• Willingness to travel extensively to member homes and facilities.

Responsibilities

• Perform adult/geriatric health assessments in home or facility setting.
• Review medical history, diagnoses, education, and follow-up plans.
• Coordinate with PCPs, case managers, and family members.
• Order and interpret point of care and lab testing as needed.
• Document findings clearly and maintain Matrix guidelines.
