Optum HouseCalls seeks a Nurse Practitioner or Physician Assistant to perform in-home health assessments for Medicare Advantage members in Benton and Franklin Counties, WA. This non-prescribing role focuses on preventive care, patient education, and coordination with primary care providers.

The position offers flexible scheduling (24–40 hours/week), no on-call requirements, and potential bilingual pay opportunities.

Formación

• NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PA: NCCPA certification and active state licensure
• Active, unrestricted license (or ability to obtain by start date)
• Valid driver’s license and reliable transportation
• Ability to work independently in home environments

Responsabilidades

• Conduct comprehensive in-home health assessments
• Review medical history and medications
• Perform physical exams, screenings, and point-of-care testing
• Identify and document diagnoses to support care planning
• Communicate findings to primary care providers
• Educate members on conditions, medications, and adherence
• Address social determinants of health and connect to resources
• Respond appropriately to urgent or emergent situations
