Optum House Calls is seeking a Nurse Practitioner or Physician Assistant to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This non-prescribing, field-based role focused on preventive care, patient education, identifying care gaps, and collaborating with primary care providers to improve health outcomes.

You will perform examinations, review medications, educate members, address social determinants of health, and connect patients with community

Qualificações

• NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• PA: NCCPA certification and active state licensure.
• Active, unrestricted state license (or ability to obtain).
• Valid driver's license and reliable transportation.

Responsabilidades

• Conduct comprehensive in-home health assessments.
• Review medical history and medications.
• Perform physical exams, screenings, and point-of-care testing.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers.
• Educate members on disease management, medications, and treatment compliance.
• Address social determinants of health and connect members with community resources.
• Recognize and respond appropriately to urgent or emergent situations.
