Job summary

OptumCare is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage and other health plan members. This non-prescribing, field-based role focuses on preventive care, health education, and closing care gaps in collaboration with primary care providers.

Qualifications

• NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PA: NCCPA certification and active state licensure
• Active unrestricted license (or ability to obtain by start date)
• Valid driver’s license and reliable transportation
• Ability to perform assessments in a variety of home environments

Responsibilities

• Conduct comprehensive in-home health assessments
• Review medical history and medications
• Perform physical exams, screenings, and point-of-care testing
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to primary care providers
• Educate members on disease management, medications, and treatment compliance
• Address social determinants of health and connect members with community resources
• Recognize and respond appropriately to urgent or emergent situations
